Registered Nurse (RN) - Diabetes Care and Primary Care

Position Summary: We are seeking a compassionate and dedicated Registered Nurse (RN) with expertise in diabetes management and primary care. The RN will play a critical role in educating, supporting, and empowering patients to manage their diabetes effectively, as well as providing primary care services. The ideal candidate will have strong clinical skills, experience in chronic disease management, and a passion for proactive patient education.

Location: Downtown Saint Louis, MO and/or Florissant, MO

Pay Range: $79,000 to $104,000

Responsibilities:
  1. Patient Assessment and Care:
  • Conduct comprehensive nursing assessments, health histories, and physical exams for patients with diabetes and other chronic illnesses.
  • Develop individualized care plans in collaboration with patients, families, and the healthcare team.

2. Diabetes Management:

  • Provide education and support to patients on monitoring blood glucose levels, understanding medication regimens, and the importance of lifestyle modifications (diet, exercise).
  • Assist in adjusting insulin therapy based on protocols and in coordination with the healthcare provider.
  • Conduct regular follow-ups to monitor patient progress and adjust care plans as necessary.

3. Primary Care Services:

  • Deliver routine primary care services such as immunizations, health screenings, and management of common acute and chronic conditions.
  • Collaborate with physicians and other healthcare professionals to deliver comprehensive patient care.

4. Patient Education and Counseling:

  • Educate patients and families about disease processes, prevention practices, and treatment plans.
  • Encourage self-management and adherence to care plans, promoting a proactive approach to health.

5. Team Collaboration and Communication:

  • Work closely with doctors, dietitians, and diabetes educators to coordinate patient care and ensure effective communication across the healthcare team.
  • Participate in multidisciplinary team meetings to review patient progress and update plans of care.

6. Documentation and Reporting:

  • Maintain accurate and comprehensive patient records in compliance with regulatory standards.
  • Track and report patient outcomes and participate in quality improvement initiatives.
Qualifications
  • Current Registered Nurse (RN) license in Missouri
  • Bachelor of Science in Nursing (BSN) preferred.
  • Certified Diabetes Educator (CDE) credential or willingness to obtain certification within a specified timeframe or equivalent experience
  • Prefer a minimum of 2 years of nursing experience, with at least 1 year in diabetes management and/or primary care.
  • Excellent clinical assessment, critical thinking, and patient care skills.
  • Proficient in electronic health records (EHR) and other healthcare technology.
  • Strong interpersonal and communication skills to effectively educate and motivate patients.
  • Ability to work independently as well as part of a collaborative team.

Working Conditions

Clinic setting with regular working hours but may require occasional evening or weekend hours to accommodate patient needs.
